Making a Claim

To make a claim, you will need to complete a claims form in conjunction with your vet and send it to our underwriter, Covéa.

There are two different forms to download:

• For a new condition that has never been claimed for before, download and complete this form.
• For a condition that has been claimed for before, download this one.

Next, either you or the vet can send the completed claims form to Covéa either by email or post.
